UPDATE - NEW YORK COSMETOLOGY LICENSES INFORMATION

General

If you marry, get divorced, lose your spouse, change your name for a personal reason, or if you move to a new house, you must notify the Division of Licensing Services of Department of State of New York for them to update their records. These changes are called “amendments” and there are several types:

  • Contact information amendment which includes:
    • Social Security number
    • Date of Birth
    • Email Address
    • Phone Number
  • Address change amendment
  • Duplicate license amendment
  • Name change amendment

Use these amendments to correct information that was wrong on your first application or data that has changed since. Once licensed, you will receive correspondence or requests from the Department. Moreover, they will send your license’s renewal notice by mail; therefore, it is vital that you maintain your information up to date.  In New York, you can change your address and name by mail but the other changes must be done online. To change the name on the cosmetology license, you must present a valid proof — marriage certificate, divorce decree, your driver's license, a non-driver's ID card, a valid passport or immigration documents — indicating your name change. There is no fee for a name change unless you request a duplicate of the license.  If you want to change the address on the cosmetology license you do not have to submit any documentation, but you must pay a $10 fee.  If you do not have an account within the NYS License Center (NYSLC), you must first register with My NY.gov.

Makeup and Cosmetics

When due to marriage, divorce, widowhood or personal reasons you change your name, or if you decide to move to a new address, make sure you notify the Division of Licensing Services of Department of State of New York for them to update the information on your makeup license. In this way, you will be sure that you do not miss any important correspondence. You can also change other pieces of information such as your email address, Social Security number and phone number. There is a $10 non-refundable fee if you change your address. The name and address can be changed online and by mail; other changes must be only made online.

To change the name or address on your makeup license online, go to NYS License Center at https://aca.licensecenter.ny.gov/aca/. Log into your account (if you do not have an account, you must create one first). Go to “Manage My Licenses” and you will see your records. Go to “Action,” click on “Amendment” and select “Name Change Amendment” or “Address change Amendment.” Finally, follow the instructions provided in the online application. If you are changing your name, you will be required to submit a marriage certificate divorce decree, court order or any other legal document as evidence. If you prefer to do the makeup license name change by mail, go to http://www.dos.ny.gov/licensing/forms.html and click on “DOS-1473 Change Notice.” Any other changes must be done online. You are not required to get a duplicate license after an amendment.

Skincare

Whether you moved to a new house or changed your name as a result of marriage, divorce, or any other personal reason, you must always notify the Division of Licensing Services of Department of State of New York. All updates can be done online by visiting the NYS License Center at: https://aca.licensecenter.ny.gov/aca/.  Keep in mind that there is a $10 non-refundable fee if you make an address change request.
If you want to change the name or address on your esthetician license by mail, go to http://www.dos.ny.gov/licensing/forms.html and click on “DOS-1473 Change Notice.” If you are looking to change the address or name on your esthetician license, go to the NYS License Center and login to your account, if you are a new user you must create an account. Go to “Manage My Licenses” and you will see your information. Go to “Action” and click on “Amendment.” Finally, select “Name Change Amendment” or “Address Change Amendment.” Follow the indications of the online application. When changing your name, you will have to submit a legal document (e.g., marriage certificate, divorce decree) or a government issued ID as evidence of your name change. You can change your name or address by mail, but other updates must be done online. You can change your phone number, Social Security number, email, among others. A duplicate is not required after an amendment.

Hairstylist/ Barber

You must maintain an effective communication with the Division of Licensing Services of Department of State of New York; therefore, if you moved to a different location or changed your name after marriage, divorce, widowhood or any personal reason, you will have to ask the Department to update your barber license. You do not have to ask for a duplicate after an update.

For a hair license address change online, go to the NYS License Center at https://aca.licensecenter.ny.gov/aca/ and login to your account, if you are a new user you must create an account. Go to “Manage My Licenses” and you will see your information. Go to “Action” and click on “Amendment.” Select “Address Change Amendment” and follow the indications of the online application. To change it by mail go to http://www.dos.ny.gov/licensing/forms.html and click on “DOS-1473 Change Notice.” There is a $10 fee when changing the address.

If you want to change the name on your hair license online, go to the NYS License Center and follow the same procedure for the name change but select “Address Change Amendment” instead.  Remember that you must attach a legal document (e.g., marriage certificate, divorce decree) or a government issued ID to proof your name change. All other changes — such as email address, phone number or Social Security number — must be done online.

Nails

Whenever you change your name or address, you must notify the Division of Licensing Services of Department of State of New York so they can update your nail license. If you move, make sure you change the address on your nail license, so you do not miss any important correspondence sent to you by the Department. You will not be required to get a duplicate of the license after an update.

If you wish to change the name on your nail license online, you have to go to the website of the NYS License Center at: https://aca.licensecenter.ny.gov/aca/. If you do not have an account, create one and then log in. Go to “Manage My Licenses” and you will see your records. Click on “Amendment” which is under “Action.” Select “Name Change Amendment” and follow the prompts. To change it by mail, visit http://www.dos.ny.gov/licensing/forms.html and click on “DOS-1473 Change Notice.”  Keep in mind that document showing name change — such as a marriage certificate, divorce decree or court order — will be requested by the Department as evidence of your name change. You can also change the address on the makeup license, using these two methods. Just select “Address Change Amendment” instead when doing it online. Also, you will be charged a $10 fee for address updates. You can change your email address, Social Security number and other contact information online only.

Electrolysis

Electrologists — those who practice electrolysis — in New York are not required to hold a license to practice; however, you have the chance to become a certified professional electrologist if you want to increase your credibility and your earnings.  The Certified Professional Electrologist (CPE) Certification is issued by the American Electrology Association (AEA). Obtaining this certification will allow you to add the letters CPE after your name.

If you decide to become part of the AEA, you will also get membership at the New York Electrolysis Association. The main job of the AEA is to promote the highest standards in electrology. If you are already a member, you have to keep your information updated as you should do with the licensing body.  If you marry or divorce and want to you change your name as a result of marriage or divorce, or if you move to a new location and have to change your name, you will have to inform the AEA which will update your records in their database.

To change your name or address, visit the AEA’s website at http://www.professionals.electrology.com and click on “Renew your AEA Membership” found at the menu on the left.  Go to “Renew Online” and click on “Change of address.” Click on it and you will be asked to log in. Once you do it, you will be able to update your information. Here, you can also change your name.
